How do I change or correct a signed clinical document 


Once a clinical document (visit note, evaluation, plan of care, progress report or discharge) is signed, you will not be able to edit this document. You have the option to Invalidate or add an Amendment to the document

 The purpose of an amendment is to add a comment about information that is incorrect in the document. This does not allow you to edit a document after it is signed.
 
The purpose of an invalidation is to cancel/remove a document and create a new one in its place. 

To invalidate or amend a signed clinical document, ensure you are logged in to Ambiki and on the main dashboard: 

  1. On the left navigation, click on ‘Patients’.

2. Select the patient whose clinical document(s) you would like to invalidate or amend by clicking on their name.

3. Locate and open the relevant signed clinical document. 

 
4. In the top right-hand corner of the document, click on the button labelled ‘Actions’ and select either Amend or Invalidate
