We have improved the management and continuity of care for patients by enhancing how plans of care and goals are handled within our system. This release includes the following:
- Single Active Plan of Care: The system now restricts to only one active Plan of Care (POC) per patient at any given time.
- New 'Discontinued' State for POC: We have introduced a 'discontinued' state for Plans of Care.
- Automatic Termination of Previous POC: When a new POC is created, the system will automatically end the previous active POC.
- Transferable Goals Between POCs: It is now possible to move goals from one Plan of Care to another when creating a new plan.
We believe these updates provide a more structured and simplified approach to managing Plans of Care. Therapists can now efficiently manage POCs, track changes, and ensure seamless goal transitions, leading to improved patient outcomes and better care coordination.
