We have implemented several important updates to the Plan Of Care (POC) feature based on user feedback:

  1. Textboxes Removed: The "Prior Level" textbox has been removed for a cleaner interface.
  2. Impressions to Rationale for Services: Updated text to a prompt asking how the patient responded to therapy interventions and the rationale for continued services, including updates to goals and any barriers to progress.
  3. Prognosis Field: Changed "Prognosis" to "Habilitative/Rehabilitative Potential" and made it a required field.
  4. Caregiver Collaboration: Added a required field for specifying caregiver collaboration during the treatment plan development.
  5. Therapy Recommendations: Defaulted to 2X/week for 30 minutes for a duration of 26 weeks. Added boxes for duration (weeks, months) and allowed up to two different durations. Made explanations required if more than one frequency is indicated. This field is now required overall.
  6. Discharge Statement: Added a default discharge statement to the POC indicating conditions for discharge from services.
  7. Visit Frequency Statement: Added a statement clarifying that visit frequency is subject to variability due to absences, illness, and inclement weather, and that missed visits will be made up within the authorization period.
  8. Signature Statement: Added a new statement at the signature section of the POC attesting to the validity and clinical appropriateness of the Plan of Care, the goals, and the recommended services.
