How do I change or correct a signed clinical document

Once a clinical document (visit note, evaluation, plan of care, progress report or discharge) is signed, you will not be able to edit this document. You have the option to invalidate or add an amendment to the document

The purpose of an amendment is to add a comment about information that is incorrect in the document. This does not allow you to edit a document after it is signed.

The purpose of an invalidation is to cancel/remove a document. You can then create a new one to replace it (if applicable).

If you need to create a copy of a clinical document, you can clone the document.

Note: The author of a clinical document may add an amendment. To make edits to a signed document that you are not the author of, the patient will either need to be on your caseload, or you can clone the document to create a new one.

To invalidate or amend a signed clinical document, ensure you are logged in to Ambiki and on the main dashboard:

  1. On the left navigation, click on Patients.

Clicking on Patients in the left navigation.

2. Select the patient whose clinical document(s) you would like to invalidate or amend by clicking on their name.

Clicking on a patient's name to access their profile.

3. Locate and open the relevant signed clinical document.

4. In the top right-hand corner of the document, click on the button labelled Actions and select either Amend or Invalidate

Clicking on 'invalidate' or 'amend' under actions in a clinical document

The author of a clinical document may add an amendment. To make edits to a signed document that you are not the author of, the patient will either need to be on your caseload, or you will need to clone the document.

You may also clone a document to create a new copy, while pulling forward the details from the old copy.

To clone a document, you click actions, and clone to new.
