How do I automatically populate a patient profile from Intake Documents 

 
Ambiki allows patient profile data to be automatically imported from custom form intake documents. When an admin is creating a new patient profile, the required personal information can be pulled from a submitted intake form, eliminating duplicate data entry. 
 
The intake document is populated by the patient or legal guardian. When uploaded during new profile creation, it is scanned for the following marked fields: patient sex, date of birth, patient name, and time zone. The extracted data is then mapped to the corresponding patient profile fields, auto-filling them. 
 
Q: How does the intake document auto-import work when creating a new patient profile? 
 
A: Here is the process when an admin is creating a new patient profile in Ambiki: 
 
  • The patient or legal guardian fills out a custom intake document form.  
 
  • The completed intake document is uploaded by the patient/legal guardian. 
 
  • The system automatically extracts the marked field values from the uploaded intake document. For now, this only includes the following fields: patient sex, DOB, name, and time zone
 
  • These extracted values are automatically populated into the corresponding fields in the new patient profile. 
 
  • The administrator then reviews the imported profile data, makes any corrections needed, and finalizes the creation of the new profile. 
 
  • The information does not need to be manually entered twice - it is carried over directly from the submitted intake document to the new patient profile.
